INTRODUCTION: During COVID-19 pandemic in Japan, nightclubs were identified as high-risk locations for COVID-19 outbreaks, but an outbreak investigation in this setting is challenging because of the anonymous and opportunistic nature of interactions. METHODS: The joint rapid response team collected epidemiological data, conducted descriptive epidemiology to determine the characteristics of cases associated with the nightclub, and implemented countermeasures. Polymerase chain reaction (PCR) tests were performed by the Local Institute of Public Health, Kagoshima University, and several commercial laboratories. RESULTS: Between June 15 and July 20, 2020, 121 individuals tested positive for SARS-CoV-2 (59 confirmed and 62 asymptomatic) of whom 8 were nightclub staff who had no travel history of outside Kagoshima, 66 were guests, and 47 were subsequent contacts. The median age was 32 years (interquartile range: 24-43 years). One individual showed severe symptoms but there were no fatal. The epidemic curve showed one peak on June 30 and July 1 with a limited number of cases subsequently. Of the 121 cases, 116 and 5 were in individuals living in and outside Kagoshima Prefecture, respectively. Haplotype network analysis showed 5 genome-wide single-nucleotide variants between the isolates before and during this outbreak. CONCLUSIONS: There is a possibility that unidentified guests from outside Kagoshima Prefecture could infect staff who could subsequently spread the virus to guests and other staff, who were mainly a younger population. The rapid outbreak response enabled onward transmission in the community to be minimized. This outbreak investigation could provide insights for effective responses to challenging situations in future pandemic.

Although thorough chewing lowers postprandial glucose concentrations, research on the effectiveness of chewing vegetables in different forms on postprandial glucose metabolism remains limited. This study examined the effects of vegetables consumed in solid versus puree forms on postprandial glucose metabolism. Nineteen healthy young men completed two 180-min trials on separate days in a random order: the chewing trial involved the consumption of shredded cabbage with chewing and the non-chewing trial involved the consumption of pureed cabbage without chewing. Energy jelly was consumed immediately after the consumption of shredded or puree cabbage. Blood samples were collected at 0, 30, 45, 60, 90, 120 and 180 min. Circulating concentrations of glucose, insulin, total glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic peptide (GIP) concentrations were measured from the plasma. Although plasma glucose concentrations did not differ between the trials, the plasma insulin and GIP incremental area under the curve values were higher in the chewing than in the non-chewing trial. Postprandial total GLP-1 concentrations were higher in the chewing than in the non-chewing trial at 45, 60 and 90 min. This study demonstrates that consuming shredded cabbage while chewing enhances postprandial incretin secretion but has no effect on postprandial glucose concentration.Trial registration: Clinical trial registration ID.: UMIN000052662, registered 31 October 2023.

OBJECTIVE: This study aimed to determine the effects of different energy loads on the gut microbiota composition and the rates of energy and nutrient excretion via feces and urine. METHODS: A randomized crossover dietary intervention study was conducted with three dietary conditions: overfeeding (OF), control (CON), and underfeeding (UF). Ten healthy men were subjected to each condition for 8 days (4 days and 3 nights in nonlaboratory and laboratory settings each). The effects of dietary conditions on energy excretion rates via feces and urine were assessed using a bomb calorimeter. RESULTS: Short-term energy loads dynamically altered the gut microbiota at the α-diversity (Shannon index), phylum, and genus levels (p < 0.05). Energy excretion rates via urine and urine plus feces decreased under OF more than under CON (urine -0.7%; p < 0.001, urine plus feces -1.9%; p = 0.049) and UF (urine -1.0%; p < 0.001, urine plus feces -2.1%; p = 0.031). However, energy excretion rates via feces did not differ between conditions. CONCLUSIONS: Although short-term overfeeding dynamically altered the gut microbiota composition, the energy excretion rate via feces was unaffected. Energy excretion rates via urine and urine plus feces were lower under OF than under CON and UF conditions.

INTRODUCTION: In this study, we aimed to analyze the effectiveness of enhanced preventive measures against nosocomial COVID-19 Omicron outbreaks based on those encountered. METHODS: We introduced PCR-based screening and syndromic surveillance, in addition to standard and transmission-based precautions, during a COVID-19 outbreak in three wards of Kagoshima University Hospital, a Japanese tertiary care hospital, in February 2022, amid the Omicron variant endemic. Furthermore, we analyzed the descriptive epidemiology and whole-genome sequencing (WGS) of positive SARS-CoV-2 PCR samples from this outbreak. RESULTS: PCR-based screening tests were conducted following the identification of three cases through syndromic surveillance. As a result, 30 individuals tested positive for SARS-CoV-2, including 13 inpatients, five attendant family members, and 12 healthcare workers across the three wards. Notably, no new infections were observed within eight days following the implementation of preventive measures. Among the SARS-CoV-2 genomes analyzed (n = 16; 53.3%), all strains were identified as belonged to BA.1.1 variant. Detailed analysis of descriptive and molecular epidemiology, incorporating single-nucleotide polymorphism analysis of WGS and clarification of transmission links, considering two potential entry routes to the hospital. CONCLUSIONS: Introduction of additional preventive measures, including PCR-based screening and syndromic surveillance, in addition to WGS and descriptive epidemiology, is useful for the early intervention of nosocomial outbreaks and for revealing the transmission route of the COVID-19 Omicron variant.

AIMS: This study determined the relationship between intra-individual variability in day-to-day nutrition-related lifestyle behaviors (meal timing, eating window, food intake, movement behaviors, sleep conditions, and body weight) and glycemic outcomes under free-living conditions in adults without type 2 diabetes. METHODS: We analyzed 104 adults without type 2 diabetes. During the 7-day measurement period, dietary intake, movement behaviors, sleep conditions, and glucose outcomes were assessed. Daily food intake was assessed using a mobile-based health application. Movement behaviors and sleep conditions were assessed using a tri-axial accelerometer. Meal timing was assessed from the participant's daily life record. Blood glucose levels were measured continuously using a glucose monitor. Statistical analyses were conducted using a linear mixed-effects model, with mealtime, food intake, body weight, movement behaviors, and sleep conditions as fixed effects and participants as a random effect. RESULTS: Dinner time and eating window were positively significantly correlated with mean (dinner time, p = 0.003; eating window, p = 0.001), standard deviation (SD; both at p < 0.001), and maximum (both at p < 0.001) blood glucose levels. Breakfast time was negatively associated with glucose outcomes (p < 0.01). Sedentary time was positively significantly associated with blood glucose SD (p = 0.040). Total sleep time was negatively significantly correlated with SD (p = 0.035) and maximum (p = 0.032) blood glucose levels. Total daily energy intake (p = 0.001), carbohydrate intake (p < 0.001), and body weight (p < 0.05) were positively associated with mean blood glucose levels. CONCLUSION: Intra-individual variations in nutrition-related lifestyle behaviors, especially morning and evening body weight, and food intake, were associated with mean blood glucose levels, and a long sedentary time and total sleep time were associated with glucose variability. Earlier dinner times and shorter eating windows per day resulted in better glucose control.

Little is known about the optimal daily magnesium (Mg) intake for individuals with high levels of physical activity. The aim of this study was to clarify the optimal dietary Mg intake for people with high levels of physical activity in a scoping review. In this review, we searched MEDLINE and Japan Medical Abstracts Society for studies published up to May 31, 2020. We conducted two searches, one for studies using gold standard measurement methods such as the balance method and factorial calculation (Search 1), and the other for studies using estimation from daily food intake (Search 2). We also performed a meta-analysis of studies that compared the Mg intake among physically active people with the Mg intake among controls. After the primary and secondary screening, 31 studies were included in the final review. All of the included studies examined professional or recreational athletes. We found no studies that examined the optimal intake of Mg using gold standard measurement methods. The Mg intake among physically active individuals was below the recommended dietary allowance in most studies. In five studies that conducted meta-analyses, physically active individuals had significantly higher intakes of Mg than controls, although these levels were still below the recommended dietary allowance. The present review revealed that evidence regarding the optimal daily magnesium intake is currently scarce, and further studies are needed.

Objective: This study examined the effects of a single and chronic oral intake of l-arginine supplementation on blood ammonia concentration and exercise performance. Methods: Sixteen healthy young men (mean ± standard deviation, 23 ± 3 years) participated in a randomised, double-blind, cross-over, placebo-controlled study. For the acute trials, the participants consumed 200 mL of water containing either l-arginine (5 g) or placebo (dextrin; 5.5 g) and performed cycling exercise at 75% of heart rate reserve for 60 min, followed by a 15-min cycling performance test. For the chronic trials, the participants continued to consume each designated supplement twice a day for another 13 days, and then repeated the same protocol as the acute trials at day 15. After a 14-day washout period, the participants changed the supplement and repeated the same protocol as above. Results: Plasma ammonia concentrations were lower in the chronic arginine trial than those in both acute placebo (mean difference - 4.5 µmol/L) and acute arginine (mean difference - 5.1 µmol/L) trials (p < 0.05). There was no difference in plasma ammonia concentration between the chronic arginine and chronic placebo trials (mean difference - 1.2 µmol/L). No differences were found in mean power output during the performance test between the chronic arginine and chronic placebo trials (mean difference 0.5 W) or between the acute arginine and acute placebo trials (mean difference 0.0 W). Conclusions: An acute and chronic oral intake of l-arginine supplementation did not attenuate exercise-induced increases in ammonia accumulation or had no significant impact on cycling performance.

PURPOSE: Although acute prolonged strenuous exercise has been shown to increase markers of gastrointestinal permeability and damage, little is known regarding the efficacy of nutritional supplement interventions on the attenuation of exercise-induced gastrointestinal syndrome. This study addressed the effects of oral amino acid supplementation on markers of gastrointestinal permeability and damage in response to exercise. METHODS: Sixteen active men aged 22.7 ± 2.6 years (mean ± standard deviation) completed placebo or cystine and glutamine supplementation trials in random order. Participants received either a placebo or cystine and glutamine supplements, three times a day for 5 days, separated by a 2-week washout period. On day 6, participants took their designated supplements 30 min before running at a speed corresponding to 75% of maximal oxygen uptake for 1 h, followed by a 4-h rest period. Blood samples were collected pre-exercise, immediately post-exercise, 30 min post-exercise, and 1, 2 and 4 h post-exercise on day 6. The plasma lactulose to mannitol ratio (L:M) and plasma intestinal fatty acid-binding protein (I-FABP) were used as markers of gastrointestinal permeability and damage, respectively. RESULTS: Plasma L:M (linear mixed model, coefficient ± standard error: - 0.011 ± 0.004, P = 0.0090) and changes (i.e., from pre-exercise) in plasma I-FABP (linear mixed model, - 195.3 ± 65.7 coefficient ± standard error (pg/mL), P = 0.0035) were lower in the cystine and glutamine supplementation trial than in the placebo trial. CONCLUSION: Oral cystine and glutamine supplementation attenuated the markers of gastrointestinal permeability and damage after 1 h of strenuous running in young men. The present study examined the effects of different temperatures of protein-containing drink after exercise on subsequent gastric motility and energy intake in healthy young men. Twelve healthy young men completed three, 1-d trials in a random order. In all trials, the subjects ran on a treadmill for 30 min at 80% of maximum heart rate. In exercise + cold drink (2°C) and exercise + hot drink (60°C) trials, the subjects consumed 300 ml of protein-containing drink (0·34 MJ) at 2°C or 60°C over a 5-min period after exercise. In the exercise (i.e. no preload) trial, the subjects sat on a chair for 5 min after exercise. Then, the subjects sat on a chair for 30 min to measure their gastric motility with an ultrasound imaging system in all trials. Thereafter, the subjects consumed a test meal until they felt comfortably full. Energy intake in the exercise + hot drink trial was 14 % and 15 % higher than the exercise (P = 0·046, 95% CI 4·010, 482·538) trial and exercise + cold drink (P = 0·001, 95% CI 160·089, 517·111) trial, respectively. The frequency of the gastric contractions in the exercise + hot drink trial was higher than the exercise (P = 0·023) trial and exercise + cold drink (P = 0·007) trial. The total frequency of gastric contractions was positively related to energy intake (r = 0·386, P = 0·022). These findings demonstrate that consuming protein-containing drink after exercise at 60°C increases energy intake and that this increase may be related to the modulation of the gastric motility.

Slow eating, which involves chewing food slowly and thoroughly, is an effective strategy for controlling appetite in order to avoid being overweight or obese. Slow eating also has the effect of increasing postprandial energy expenditure (diet-induced thermogenesis). It is still unclear whether this is due to oral stimuli; that is, the duration of tasting food in the mouth and the duration of chewing. To investigate the effects of oral stimuli on diet-induced thermogenesis in 11 healthy normal weight males, we conducted a randomized crossover study comprising three trials: (1) drinking liquid food normally, (2) drinking liquid food after tasting, and (3) adding chewing while tasting. Oral stimuli (i.e., the duration of tasting liquid food in the mouth and the duration of chewing) significantly increased diet-induced thermogenesis after drinking liquid food. This result demonstrates that the increase in diet-induced thermogenesis is due to oral stimuli rather than the influence of the food bolus. Increased diet-induced thermogenesis induced by chewing and taste stimuli may help to prevent overweight and obesity.

BACKGROUND/OBJECTIVE: Gum chewing while walking increases walking distance and energy expenditure in middle-aged male and female individuals. This study aimed to examine the effects of gum chewing while walking on walking distance and energy metabolism in male and female individuals of various age groups. METHODS: Fifty participants (25 male and 25 female individuals) aged 22-69 years completed two trials in a random order. In the gum trial, participants walked at a natural pace for 15 min while chewing two gum pellets (1.5 g, 3 kcal per pellet) following a 50-min rest period. In the tablet trial, participants rested for 50 min before walking, and the participants then walked at a natural pace for 15 min after ingesting two pellets of tablet containing the same ingredients with the exception of the gum base. The walking distance, step count, walking speed, stride, heart rate, energy expenditure, and respiratory exchange ratio were measured. RESULTS: Walking distance, step count, walking speed, heart rate, and energy expenditure during walking were significantly higher in the gum trial than in the tablet trial. In participants aged ≥40 years, walking distance, walking speed, stride, heart rate, and energy expenditure during walking were significantly increased during the gum trial compared with those during the tablet trial. CONCLUSION: The study findings demonstrated that gum chewing while walking increased walking distance and energy expenditure in both male and female individuals.

Detection of severe fever with thrombocytopenia syndrome (SFTS) virus (SFTSV) during the early phase of the disease is important for appropriate treatment, infection control, and prevention of further transmission. The reverse transcription loop-mediated isothermal amplification (RT-LAMP) is a nucleic acid amplification method that amplifies the target sequence under isothermal conditions. Here, we developed an RT-LAMP with a novel primer/probe set targeting a conserved region of the SFTSV L segment after extraction of viral RNA (standard RT-LAMP). Both the Chinese and Japanese SFTSV strains, including various genotypes, were detected by the standard RT-LAMP. We also performed RT-LAMP using the same primer/probe set but without the viral RNA extraction step (called simplified RT-LAMP) and evaluated the diagnostic efficacy. The sensitivity and specificity of the simplified RT-LAMP were 84.9% (45/53) and 89.5% (2/19), respectively. The simplified RT-LAMP can detect SFTSV in human sera containing >103.5 copies/mL viral RNA. The two RT-LAMP positive but quantitative real-time reverse transcription-polymerase chain reaction (RT-PCR) negative samples were positive in the conventional RT-PCR, suggesting that there was no false positive reaction in the RT-LAMP. Both the standard and simplified RT-LAMP are useful for detecting the SFTSV genome in patients during the early phase of the disease.

OBJECTIVE: This study examined the chronic effect of increased physical activity on postprandial triglycerides in older women. METHODS: Twenty-six women, aged 72 ± 5 years (mean ± SD), participated in this study. Participants in the physical activity group (n = 11) were asked to increase their activities above their usual lifestyle levels for 12 weeks. Participants in the control group (n = 15) maintained their usual lifestyle for 12 weeks. All participants rested and consumed a standardized breakfast after a 24-h period of physical activity avoidance at baseline, 4 weeks, and 12 weeks. Blood samples were collected in the fasted state (0 h) and at 2, 4, and 6 h after breakfast. RESULTS: The average increased time spent in self-selected activities per day was 1.1 ± 19.3 min over the 12 weeks compared with the baseline in the physical activity group. There was no difference in the postprandial time-averaged triglyceride area under the curve at baseline (1.59 ± 0.81 vs. 1.39 ± 0.67 mmol/L, p = 0.515) or over the 12-week intervention (1.78 ± 1.00 vs. 1.31 ± 0.67 mmol/L, p = 0.212) between the physical activity and control groups. CONCLUSION: Postprandial triglyceride concentrations were not reduced after performing self-selected activities under free-living conditions in older women when these responses were determined 24 h after the last physical activity bout. PURPOSE: Dietary replacement of an acute exercise-induced energy deficit offsets the postprandial triglyceride (TG)-lowering effect of exercise in young boys and middle-aged men. It is unclear whether these findings are observed when exercise is accumulated in older adults. This study examined the effect of accumulating short bouts of exercise, with and without dietary replacement of an exercise-induced energy deficit, on postprandial TG in older women. METHODS: Seventeen older women (≥ 65 years) underwent three, 8-h trials: (1) control, (2) accumulated walking and (3) accumulated walking with energy replacement. During the control trial, participants rested for 8 h. The accumulated walking trials comprised twenty 1.5 min brisk walking bouts performed at a pre-determined self-selected pace separated by 15 min seated rest. In each trial, participants consumed a standardised breakfast and lunch. The breakfast in the accumulated walking with energy replacement trial included replacement of the energy deficit (0.62 MJ, 149 kcal) induced by exercise. Venous blood samples were collected fasted and at 2, 4, 6 and 8 h after breakfast. RESULTS: Time-averaged postprandial serum TG concentrations over 8 h were lower after accumulated walking than control and accumulated walking with energy replacement (mean ± SD: 1.46 ± 0.93 vs 1.71 ± 1.01 vs 1.60 ± 0.98 mmol/L, respectively: main effect of trial p = 0.017). There was little difference between control and accumulated walking with energy replacement. CONCLUSIONS: Replacing the energy expenditure induced by accumulating 30 min of brisk walking in short (1.5 min) bouts diminishes the postprandial TG-lowering effect in older women.

This study investigated the effect of high-intensity cycling re-warm up (RW) within a very short time-frame on the subsequent intermittent sprint performance. Twelve active males completed three trials in random order: control (CON); 3-min RW at 30% of maximal oxygen uptake (VO2max) (RW30); and 1-min RW at 90% of VO2max (RW90). During the experimental trials, participants performed 40-min intermittent cycling exercise followed by 15-min rest. During the rest period, participants completed CON, RW30, or RW90. After the rest period, participants performed the Cycling Intermittent-Sprint Protocol (CISP), which consisted of 10-s rest, 5-s maximal sprint, and 105-s active recovery with the cycles repeated over 10 min. The mean work during sprint for the CISP was significantly higher in both RW trials than in the CON trial (mean±standard deviation; CON: 3539±698 J; RW30: 3724±720 J; RW90: 3739±736 J; p<0.05). The mean electromyogram amplitude during the sprint for the CISP was higher in the RW30 trial than in the CON trial; however, there was no significant difference between the two trials (p=0.06). The mean median frequency during sprint for the CISP was significantly higher in the RW90 trial than in the other trials (p<0.05). Rectal temperature did not differ among the three trials. Oxygenated haemoglobin during the initial 30 s of the CISP was significantly higher in the RW90 trial than in the CON trial (p<0.05). Compared with seated rest, RW, irrespective of whether it comprised 1-min at 90% of VO2max or 3-min at 30% of VO2max, increased the subsequent intermittent sprint performance.

Pre-meal drink ingestion is an effective method of controlling energy intake in humans. However, no studies have addressed the influence of differences in the volume of drink intake on gastric motility and energy intake. The purpose of the present study was to examine the effects of differences in the volume of drink intake before a meal on subsequent gastric motility and energy intake in healthy young men. Twelve men completed two, one-day trials in a random order. Subjects visited the laboratory after a 10-h overnight fast and consumed the nutrient drink (0.84 MJ) in either 100 mL or 600 mL quantities over a 5-min period. Then, the subjects sat on a chair for over 2 h to measure their cross-sectional gastric antral areas and gastric contractions with an ultrasound imaging system. Thereafter, the subjects consumed a test meal until they felt completely full. Energy intake was calculated from the amount of food consumed. Energy intake in the 600 mL trial was 12% higher than the 100 mL trial (5.1 ±â€¯1.3 vs. 4.6 ±â€¯1.4 MJ, P = 0.046). The antral area (P = 0.046) and the frequency of the gastric contraction (P = 0.001) over 2 h after consuming the nutrient drink were higher in the 600 mL trial than the 100 mL trial. These findings demonstrated that consumption of a 600 mL nutrient drink increased energy intake. The modulation of gastric motility might have some effects on energy intake.

PURPOSE: Although immediate pre-meal water ingestion has been shown to reduce energy intake in healthy young men, no studies are available regarding potential mechanisms underlying the effect of energy intake in response to different temperatures of pre-meal water ingestion. This study examined the effects of consuming different temperatures of water on gastric motility and energy intake in healthy young men. METHODS: Eleven young men were completed three, 1-day trials in a random order. Subjects visited the laboratory after a 10-h overnight fast and consumed 500 mL of water at 2 °C, 37 °C, or 60 °C in 5 min. Then, subjects sat on a chair over 1 h to measure the cross-sectional gastric antral area and gastric contractions using the ultrasound imaging systems. Thereafter, subjects consumed a test meal until they felt completely full. Energy intake was calculated from the amount of food consumed. RESULTS: Energy intake in the 2 °C (6.7 ± 1.8 MJ) trial was 19% and 26% lower than the 37 °C (7.9 ± 2.3 MJ, p = 0.039) and 60 °C (8.5 ± 3.2 MJ, p = 0.025) trials, respectively. The frequency of the gastric contractions after 1-h consuming water was lowered in the 2 °C trial than the 60 °C trial (trial-time interaction, p = 0.020). The frequency of gastric contractions was positively related to energy intake (r = 0.365, p = 0.037). CONCLUSIONS: These findings demonstrate that consuming water at 2 °C reduces energy intake and this reduction may be related to the modulation of the gastric motility.

BACKGROUND/OBJECTIVE: Isomaltulose is a disaccharide with a low glycaemic index and plays a role in maintaining postprandial glucose. The maintenance of glucose availability during prolonged exercise has been shown to enhance exercise performance. The present study compared the effects of pre-exercise isomaltulose versus maltodextrin ingestion on gastric parameters and cycling performance in young men. METHODS: Fourteen young men (mean ±â€¯S.D., age 23 ±â€¯2 years) performed 60 min of continuous cycling at 75% of maximum heart rate followed by a 15-min exercise performance test while ingesting a 500-mL of water containing 100 mg of 13C-sodium acetate with either 50 g of isomaltulose or 50 g of maltodextrin. Gastrointestinal discomfort was assessed periodically using an 11-point visual analogue scale throughout the study. The gastric emptying rate was evaluated periodically with the 13C-sodium acetate breath test. For the exercise performance test, participants were instructed to pedal a cycle ergometer, exerting as much effort as possible at a self-selected pace. RESULTS: Plasma glucose and insulin concentrations measured at 30 min after ingestion were lower in the isomaltulose trial than in the maltodextrin trial. There were no differences in mean power output during the exercise performance test, gastric emptying rate or the subjective feelings of gastrointestinal discomfort between both trials. CONCLUSION: Under the current exercise protocol, pre-exercise ingestion of isomaltulose compared with maltodextrin provided no additional benefit relative to gastric emptying or aerobic exercise performance. Both isomaltulose and maltodextrin ingestion did not influence gastrointestinal distress during 60 min of cycling and performance test.
